BODOGWU FORM M application is a replacement for Form M application via single window portal. BODOGWU FORM M APPLICATION: OVERVIEW
- Firstly, users log in with their email and password to the BODOGWU portal to apply for Form M. ​
- Secondly, applicant must fill in the mandatory fields, including beneficiary and goods details. ​
- Thirdly, necessary documents like proforma invoice, SON PC, NAFDAC certificate, etc. are to be in place for upload. ​
- Fourthly, applicant can save and submit Form M for bank validation.
BODOGWU FORM M APPLICATION: systematic guide
Step 1: User login on the portal using the email address and password, to open the application for the Form M, which is on the dashboard. Upon login, the Homepage will display as seen below.
Also, step 2: Click on the add Form M button to display the Form M application page. Here you shall fill the following pages
- The header page
- Name and parties: this refers to importer and exporters details
- Transport: this is where you state shipping mode: sea, air or road. In addition, you will state ports of shipping and arrival, and the Customs command.
- Financials is where you fill in bank details, source of funds, currency of transaction, exchange rate and all financial details.
- Goods page is where you enter details of the goods of import, including the description, quantity, price, etc.
- Attachment page allows you to add SON PC, NAFDAC permit, NERC and other import permits, as well as the proforma invoice and the insurance certificate.

HEADER PAGE
Fill all the mandatory fields under each Tab. You are to select if transaction is valid for forex or not. In addition, this is where you select applicant’s dealer bank and bank address and branch. Click next to move to names and parties after filling this page.
BENEFICIARY
Here you notice that the importer’s details are already in place as imported from the corporate affairs commission after registration. Click on the Beneficiary button and type in the Beneficiary name to show the name from the drop down menu.
If the Beneficiary details do not exist in the drop down, click on the Add beneficiary. Enter required information as in the proforma invoice accurately. Click the add button once you have added all details of the exporter. Then, press next to go to the transport page.
TRANSPORT
Fill in country of supply of goods, ports of loading and discharge. Equally, fill in mode of transport, as well as shipment date if you have it. Otherwise, ignore items that are not compulsory and move to the next – financials.
FINANCIALS
Fill in the details as displayed below and move to the next – goods.
GOODS
To add goods, click on Goods tab, fill the general goods description & then click on Add item.
- Fill in all the necessary details and click on Add. Repeat the same for all items in the invoice. After successfully adding item(s), the Goods section display an overview of the item(s) as seen. Click next to go to add attachment.
ATTACHMENT
Click on the Tab Attachment to add necessary documents inclusive of Proforma Invoice, Insurance Certificate, NAFDAC certificate, Product Certificate etc.
With all documents properly attached, click save to store all information on the portal.
Check carefully, and save.
To submit the already saved Form M, click on Submit button, then confirm by clicking the Yes button on the pop-up window.
Lastly, submit the Form M for processing by the bank. You will get prompt that Form M is submitted successfully, awaiting validation by the Bank.
